Internship and Transfer Credit Forms 

SAS Transfer Course Departmental Evaluation Form

This form would be used if a student has taken a course at another institution (not a community college) that would like the course to be evaluated for Sociology credit.  Please provide us with the syllabus for the course. 

SAS Transfer Credit Preapproval Form

This form would be used if a student is planning on taking a Sociology course at another institution.  Please provide the department with the syllabus for the class.

Internship for Credit Form

This form is used for students to receive course credit for completing an internship. Please review relevant policies on the department's internship page to ensure the internship is eligible.
